Health Plans
24-7 Health Connect technology and comprehensive service is normally covered by Medicare and other commercial health plans, with little to no deductible for the patient. Your practice’s billing and reimbursement occurs monthly, beginning with the patient’s orientation session for their wireless device(s) and 24-7 Health Connect Center monitoring.
Health Coaching
Health Coaching helps Members develop healthier lifestyle habits. A Health Coach works with each individual to design a personalized action plan and to provide education, encouragement, and accountability. We help Members navigate the platform, monitor their progress, and celebrate their milestones.
What does Health Coaching involve?
Members work with a Health Coach, one-on-one, for 6-12 months
Members participate in a live, virtual orientation to set goals and design an action plan with their Health Coach
Members receive accountability and support through regular contact with their Health Coach
Members can choose to take part in online educational opportunities
Members are able to consult with supplemental healthcare professionals, as needed (i.e., dietician, physical therapist, etc.)
